Buchner Clinic Pre-Paid Package and Annual Membership Agreement and Terms

This agreement is between Buchner Clinic PLLC, William Buchner Jr MD and the patient purchasing and signing this document in the document box below.

Annual Membership Packages:
The Provider agrees to provide the patient with the services outlined in the package or membership they purchased at the discounted rate applied in the purchase transaction and annual agreement. An annual membership plan is charged a monthly fee and the listed services must be utilized during that billing cycle. Unused appointment services are not banked or carried forward. Annual memberships will include a discount of 50% off any additional service of equal of lesser value provided during that billing cycle. An annual plan agreement must remain active for 11 months and be paid on time through automatic recurring billing. Early termination of an annual membership plan will be charged a fee of $399 to discontinue before 11 months of service.

Pre-paid Service Packages:
The services purchased can be utilized by the patient for the listed service at any time up to 12 months after the purchase and cannot be substituted for other service types. 12 months following the purchase of a prepaid package, any unused services will expire and cannot be used.

Pre-paid package and annual memberships are non-refundable after the 1st service is performed and completed. Upon request at the time of a service not included in their package-membership, patients who have purchased a package will qualify for a 5-20% discount on any other provider-visit based service needed during the 12 month period after their package was purchased. The pre-paid package and annual membership discounts do not include lab fees, IV treatments, procedures or supplements in our clinic, but the member is eligible for a 20% discount on any supplements purchased through our Full Scripts Dispensary during the 12 month period.

The patient agrees to pay for their prepaid package in full prior to receiving its benefits or agrees to paying a monthly fixed fee for an annual membership. The Provider reserves the right to terminate the Package-Membership Program at any time for any reason.

The Provider reserves the right to amend the Membership and its associated fees at any time. The Member agrees to provide the Provider with up-to-date contact information so that the Provider can communicate with the Member.

The patient agrees to keep all appointments and to inform the provider at least 1 business days in advance of any cancellations. Appointments not cancelled with-in 24 hour notice will incur a $25 no-show fee.

The patients agrees they will be located in South Dakota, Michigan or California at the time of any video visit. Patients can also have their appointment in person at our Sioux Falls office.

Unused prepaid appointments will expire if not used within 12 months and no refunds will be given for unused appointments.

The Member and the Provider agree to the terms of this Membership Agreement.
