Acute Care Membership Package $49/mo
Annual membership for patients wanting monthly check-ins for our acute care visit types including basic medication refills, acute visit types, and follow up on routine family medicine based medical concerns. Plan includes 1 monthly acute care visit type appointment and 50% off additional acute care appointment types. Members will receive discounts to our FullScript supplement dispensary and VIP appointment time slots.
Hormone Optimizer Membership $99/mo
Annual membership for patients wanting a monthly check-in for hormone management and optimization including peptide therapies, weight loss coaching, supplement recommendations, sexual health and longevity focused care. Plan includes 1 monthly hormone based appointment type and 50% off any additional hormone visits or acute care visits. Members will receive discounts to our FullScript supplement dispensary and VIP appointment time slots.
Enviro-PLUS Membership $149/mo
Annual membership for patients being treated for environmentally acquired illnesses. Plan includes 1 monthly appointment and 50% off additional environmental or acute care appointment types. Members will receive discounts to our FullScript supplement dispensary and VIP appointment time slots.
Functional Med VIP Membership $199/mo
Annual membership for patients wanting a monthly check-in for functional medicine services including hormone management, weight loss coaching, supplement recommendations, and management of environmental illnesses. Plan includes 1 monthly functional medicine appointment and 50% off any additional visit type we offer. Members will receive discounts to our FullScript supplement dispensary and VIP appointment time slots.
